As for H2 antagonists, Garnett told the audience, “One of the most frequent mistakes I see with GERD is that they don’t dose H2 antagonists high enough.” He explained that the dose for PUD is usually about half that required for GERD, and he reminded pharmacists that GERD has a different dosing schedule than PUD. Garnett said that in mild to moderate disease, H2 antagonists provide symptomatic relief and promote healing. But not in severe disease, he said. He also mentioned a recent finding that suggested that tolerance to H2 antagonists may develop when treating gerd complications.

Garnett said that omeprazole relieves symptoms, facilitates healing, and is effective in patients refractory to H2 antagonists. He explained that by inhibiting the hydrogen/potassium ATPase pump, omeprazole completely suppresses acid secretion, and noted a meta-analysis that compared omeprazole with H2 antagonists and showed that omeprazole healed people more quickly and gave them more symptomatic relief.
