I’m not familiar with state-funded disability for family leave. For California, you may have to call the California Labor & Workforce Development Agency for info.

This is another page from the same site with forms for unemployment benefits, disability benefits, etc.

The disability would be the late-term pregnancy, and the family leave would be once you start your family. Logically the disability would therefore occur first before the family leave. Depending on your employer, you will likely be put on light-duty to accommodate your disability eliminating any need to have to apply for such. Of course you’re going to want to take advantage of the PFL soon after your new addition. Lots of information regarding the PFL can be found here: Congratulations!
